Beller, L et al. (2015) Palliative Pharmacological Sedation For Terminally Ill Adults. Cochrane Database of Systematic Reviews, 1.  Link to Full Text

Abstract: Terminally ill people experience a variety of symptoms in the last hours and days of life, including delirium, agitation, anxiety, terminal restlessness, dyspnoea, pain, vomiting, and psychological and physical distress. In the terminal phase of life, these symptoms may become refractory, and unable to be controlled by supportive and palliative therapies specifically targeted to these symptoms. Palliative sedation therapy is one potential solution to providing relief from these refractory symptoms. Sedation in terminally ill people is intended to provide relief from refractory symptoms that are not controlled by other methods. Sedative drugs such as benzodiazepines are titrated to achieve the desired level of sedation; the level of sedation can be easily maintained and the effect is reversible.

Good, P et al (2014) Medically Assisted Hydration For Adult Palliative Care Patients. Cochrane Database Systematic Review, 4, Link to Full Text

Abstract: Many palliative care patients have reduced oral intake during their illness. The management of this can include the provision of medically assisted hydration with the aim of prolonging the life of a patient, improving their quality of life, or both. This is an updated version of the original Cochrane review published in Issue 2, 2008, and updated in February 2011.

Abstract: Many palliative care patients have a reduced oral intake during their illness. The management of this can include the provision of medically assisted nutrition with the aim of prolonging the length of life of a patient, improving their quality of life, or both. This is an updated version of the original Cochrane review published in Issue 4, 2008.

Gurusamy SS et al (2014) Resection Versus Other Treatments For Locally Advanced Pancreatic Cancer. Cochrane Database of Systematic Reviews, 2.  Link to Full Text

Abstract: Pancreatic cancer is an aggressive cancer. Resection of the cancer is the only treatment with the potential to achieve long-term survival. However, a third of patients with pancreatic cancer have locally advanced cancer involving adjacent structures such as blood vessels which are not usually removed because of fear of increased complications after surgery. Such patients often receive palliative treatment. Resection of the pancreas along with the involved vessels is an alternative to palliative treatment for patients with locally advanced pancreatic cancer.

Abstract: Palliative radiotherapy to the chest is often used in patients with lung cancer, but radiotherapy regimens are more often based on tradition than research results. This is an update of a Cochrane review first published in 2001 and previously updated in 2006.

Storrar, Jet al.  (2014) Droperidol For Treatment Of Nausea And Vomiting In Palliative Care Patients. Cochrane Database of Systematic Reviews 11.

Link to Full Text

Abstract: This is an updated version of the original Cochrane review published in Issue 10, 2010, on droperidol for the treatment of nausea and vomiting in palliative care patients. Nausea and vomiting are common symptoms in patients with terminal illness and can be very unpleasant and distressing. There are several different types of antiemetic treatments that can be used to control these symptoms. Droperidol is an antipsychotic drug and has been used and studied as an antiemetic in the management of postoperative and chemotherapy nausea and vomiting.

Reports & Statistics

Hospice UK and National Council for Palliative Care (2014) Deprivation of Liberty Safeguards: An Update for Hospices.

Abstract: A briefing for all staff and volunteers providing hospice care as the people they support are likely to be protected by the Mental Capacity Act (2005) and the Deprivation of Liberty Safeguards (DoLS).

Marie Curie Cancer Care (2014) Death And Dying In Wales: An Analysis Of Inconsistencies In Access To Specialist Palliative Care And Hospital Activity In The Last Year Of Life.

Abstract: This report analyses data from the NHS Wales Informatics Service (NWIS) regarding people who are recorded as having received specialist palliative care prior to their death for everyone who died in Wales in 2012. It reveals some striking findings relating to admissions and bed days in the last year of life and access to palliative care according to the cause of death and age, yet a smaller role for deprivation than anticipated.

National Council for Palliative Care (2014) Life After Death: Six Steps to Improve Support in Bereavement

Abstract: A joint publication from NCPC, Dying Matters and the National Bereavement Alliance. This briefing identifies six steps to address bereavement as a major public policy issue.

National Council for Palliative Care (2014) NCPC Specialist Palliative Care (SPOC) Workforce Study 2013: SPC Longitudinal Survey Of English Strategic Clinical Networks.

Abstract: The National Council for Palliative Care (NCPC) has been conducting surveys to form a picture of the current make up of the specialist palliative care workforce. This includes data about palliative medicine consultants and SpRs, nurses and Allied Health Professionals. They have run surveys in 2005, 2007, 2008, 2009 and 2010, enabling them to identify national trends and priorities for workforce development. The survey provides the only comprehensive overview of the specialist palliative care workforce covering both the NHS and voluntary sector. This is a valuable resource for commissioners and providers to identify gaps and priorities for their local workforce planning and development. This report identifies some of the key findings from the 2013 collection results and then highlights some of the trends over the years the survey has been running (2005-13). The report has been produced by Mouchel plc and has been supported by the National End of Life Care Intelligence Network (NEoLCIN) and Public Health England (PHE).

National Council for Palliative Care (2014) The End of Life Care Strategy: New Ambitions.

Abstract: 2013 was the halfway point of the 10-year 2008 End of Life Care Strategy. To mark that, NCPC held a conference “Refreshing the strategy. The next five years for end of life care: what do we need to do?”. This report includes reflections from the conference day, and also additional recommendations and thoughts about what next in end of life care. It is intended as an initial contribution and will be followed by further publications during 2014-15.

National End of Life Care Programme (2014) Planning For Your Future Care.

Planning for your future care, which was originally published in 2009 with the National End of Life Care Programme,  the Dying Matters coalition and the University of Nottingham, and revised in 2012. This new edition has been updated and revised for 2014, serves as an easy-to-read introduction to advance care planning, and is one of the most requested booklets at NCPC.

NHS Scotland (2014) Scottish Palliative Care Guidelines.

Abstract: This guideline is an aid to clinical decision making and good practice in person-centred care for patients who are deteriorating and at risk of dying. The patient may have a new life-limiting condition, one or more advanced illnesses, or both. A decision will have been made that hospital admission or transfer to a high-dependency unit or intensive treatment unit is not appropriate. While this guideline focuses on physical symptoms, psychosocial and spiritual issues will also need to be addressed to give holistic care

NHS Wales (2014) Together For Health: End Of Life Care Delivery Plan Annual Report 2014.

Abstract: This publication of this first all-Wales annual report for end of life care is a major step towards making the NHS more accountable and visible to the people of Wales. For the first time it brings together in a single place simple, clear information on how services for those nearing the end of life are performing at a local and national level. It highlights the progress being made in Wales and identifies areas for future improvement. It demonstrates how health boards are taking local ownership through their delivery plans to improve end of life care and drive up standards of patient care in their communities.
